The fundamental way to cure BPH is to resect the hyperplastic prostate tissue by surgery, and there are several surgical methods for BPH:
Bilateral orchiectomy
Bilateral orchiectomy is a palliative surgical treatment for benign prostatic hyperplasia. Because this operation does not involve the prostaglandins deep in the body, but the testicles on the body surface. By removing the testicles, the male hormones in the body almost disappear, thus improving the symptoms of patients. This operation has the advantages of small trauma and short time, so it can be selected for some elderly and infirm patients with important organ complications.
Transurethral resection of prostate
In the past 20 years, a new surgical treatment for benign prostatic hyperplasia, namely transurethral resection of prostate, has emerged. During the operation, it is only necessary to insert a cystoscopic resectoscope from the urethra to the prostate for cutting. This operation has the advantages of small surgical trauma, short operation time and fast recovery of patients.
Applicable to open surgery
The indications of open prostatectomy for benign prostatic hyperplasia are relatively wide, which can be selected by the attending doctor in combination with hospital conditions, surgical proficiency and experience, including suprapubic prostatectomy, retropubic prostatectomy and transperineal prostatectomy.
① Suprapubic transbladder prostatectomy. The operation has a history of nearly one hundred years. After continuous improvement, it has become a relatively safe and simple operation method. In China, the most patients adopt this operation method.
② Postpubic prostatectomy. Because this surgical method does not cut the bladder, it is difficult to expose the prostate protruding out of the bladder or too small. Therefore, it is more suitable for patients with moderate prostate hyperplasia. In addition, patients with benign prostatic hyperplasia complicated with inflammation, stones and other diseases should first choose this operation to prevent infection from invading the bladder.
③ Transperineal prostatectomy. This surgical method is applicable to the hyperplasia of the lateral lobe of the prostate in the urethra, but it is more often used for the treatment of prostate cancer, and less used for the treatment of prostatic hyperplasia.
