Not easy to find early
The prostate is a gland located under the bladder and surrounding the urethra, which is normally as big as a chestnut and is unique to men. According to statistics, there are about 80000 new cases of prostate cancer in China every year, mainly in the elderly over 60 years old. Because the prostate position is hidden, the cancerous sites are mostly located in the posterior lobe perizone, and the early stage will not oppress the urethra and cause dysuria and other manifestations, so there are often no symptoms in the early and middle stages of the disease, which is difficult to arouse the vigilance of patients; Even if there is discomfort, such as dysuria, it is often mistaken for the performance of old age, or prostate hypertrophy, which delays early diagnosis and treatment. When the tumor increases to a certain extent and produces pressure on the urethra, it is often not early. In our hospital, many patients first developed bone pain and found distant bone metastases. After examination, it was found that the primary focus was from prostate cancer.
Men over 50 years old should have a special examination every year, including digital rectal examination, prostate specific antigen (PSA) and transrectal ultrasound examination, and prostate puncture biopsy should be performed for suspicious cases.
The prognosis of early prostate cancer is good
For early prostate cancer, the best treatment is radical surgery, that is, when the canceration is confined to the prostate, radical resection can achieve the goal of lifelong cure, and does not affect the quality of life of patients. For prostate cancer that breaks through the prostate capsule and has local invasion, radical surgery or castration surgery combined with endocrine therapy can be considered on the basis of neoadjuvant treatment. With the improvement of surgical level and anatomical technology, the radical operation of reserved nerves and vessels can make most patients recover their sexual function after operation. Advanced prostate cancer can be treated with anti-androgen, combined with local radiotherapy or chemotherapy according to the situation, and most of them have good curative effect.
In clinical work, we often encounter patients who have undergone transurethral prostatectomy due to benign prostatic hyperplasia. When conducting pathological examination of the removed prostate tissue after the surgery, prostate cancer is accidentally found, which is called incidental cancer. This type of prostate cancer is mostly localized, with small tumor size, good cell differentiation, slow growth, few metastasis opportunities and good prognosis.
Eat more fish and tomatoes
Because the etiology of prostate cancer is not clear, there is no effective preventive measures at present. The latest research results show that the risk of prostate cancer of people who never eat fish is two to three times higher than those who often eat fish, and the probability of prostate cancer of men with the highest serum lycopene level is 35% lower than that of men with the lowest level. These results suggest that eating more fish and tomatoes (tomatoes) has a certain role in preventing and reducing the incidence of prostate cancer.
