Psychotherapy
For patients with obvious mental illness, sexual psychological therapy can be performed alone or in collaboration with other treatment methods. But sexual psychotherapy takes time and the efficacy is uncertain.
First line treatment
Phosphodiesterase 5 is a widely distributed enzyme in the corpus cavernosum tissue of the penis, which can hydrolyze cyclophosphoric anhydride. The activity of pDE5 can block the increase of cGMp hydrolysis concentration, induce relaxation of penile blood vessels and smooth muscle of cavernous sinus, increase penile arterial blood flow, and induce penile erection. At present, there is a high selectivity for pDE sildenafil inhibitors (*), and he replied that the clinical efficiency of taking both sildenafil and vardenafil before sexual activity reaches 70-80%. The most common side effects of pDE5 inhibitors are headache, flushing, indigestion, and nasal congestion, all of which are mediated by nitric oxide cyclophosphate. 1. Treatment of vacuum negative pressure device
When using a negative pressure device, place a negative pressure ring at the base of the penis to prevent blood reflux. Negative pressure acts on the corpus cavernosum of the penis, attracting blood into the penis and causing a passive erection. Generally speaking, young patients are less likely to accept negative pressure devices. Elderly patients with erectile dysfunction who have low sexual intercourse frequency and those who prefer non-invasive and non pharmacological treatment are more likely to receive negative pressure therapy. The side effects are penile pain, numbness, and inability to ejaculate.
