Infertility caused by abnormal semen and sperm
1、 Insufficient semen volume
In general, a normal male ejaculates approximately 2-6ml per ejaculation, and the amount of ejaculation is related to the frequency of ejaculation. If the semen volume is less than 2ml each time for more than 48 hours, it is called oligospermia.
Etiology
The reasons for decreased semen include: ① obstruction of the ejaculatory duct or congenital absence of seminal vesicles; ② Pituitary or testicular interstitial cell lesions, resulting in a decrease in gonadotropins or a decrease in androgens, leading to a decrease in semen production; ③ There are infectious diseases in the genital tract, which can also cause functional damage of accessory glands, organization or fibrosis of accessory glands, and reduce semen production.
Diagnosis
To diagnose oligospermia, we must first rule out some semen omissions and retrograde ejaculation when collecting semen. After confirming the decrease in semen volume, a series of further examinations are needed to determine the location of the lesion. Generally speaking, low semen volume caused by obstruction of the ejaculatory duct or congenital absence of seminal vesicles can be accompanied by both azoospermia and fructose deficiency in semen; A decrease in semen volume without sperm deficiency may be due to pituitary or testicular interstitial lesions, resulting in a decrease in gonadotropins or androgens. When genital tract infection causes damage to the function of accessory gonad, a large number of white blood cells can appear in semen. Bacterial culture and counting can help diagnosis. For chronic diseases, the function of accessory gonad can be judged by imaging and functional enzyme examination.
2、 Prolonged semen liquefaction time
Normal semen is in a liquefied state when ejaculated, and immediately forms a gel or clot. After 5 to 20 minutes in a 37 ℃ water bath, the semen changes from a solidified state to a liquefied state, which is called semen liquefaction. If this process takes more than 30 minutes, it is called an extension of semen liquefaction time.
