If bacterial culture is also performed, a clear diagnosis and classification of prostatitis can be made, thereby understanding the direction of the next treatment.
Examination method II. Tissue puncture
The next step is to detect the nature of the prostate tumor, which is very useful for clarifying the histological classification and cytological characteristics of prostate tumors. It can greatly facilitate transrectal needle aspiration biopsy
Examination Method III: Rectal Examination
The doctor will perform a rectal examination. The male prostate is soft, plump, enlarged, and slightly tender. If the disease persists for a long time, the prostate will gradually become smaller, harder, uneven in texture, and there will be small nodules. At the same time, the male should be asked which part of the prostate has pain by pressing on it.
Laboratory examination:
1. Prostate fluid examination showed that white blood cells in prostate fluid were>10/HP, and there was a significant decrease in lecithin bodies. IgA in immunoglobulins increased, zinc content decreased, and pH increased.
2. Meares Stamey four-cup localized bacterial culture method for segmented urine and prostate fluid culture examination: before examination, fully drink water and take 10ml of initial urine (VB1); After another 200 ml of urination, take 10 ml of middle urine (VB2); Massage the prostate and collect prostate fluid (EPS); After completing urination, 10ml (VB3) of urine was sent for bacterial culture and colony count. Chronic bacterial prostatitis can be diagnosed by bacterial count VB3>VB110 times; "If VB1 and VB2 bacterial cultures are negative, and prostate fluid and VB3 bacterial cultures are positive, the diagnosis can also be determined.".
The impact of prostatitis on semen is still controversial. Many studies have shown that normal ejaculated fresh semen containing excessive microorganisms can reduce sperm motility, but this only occurs when there are large amounts of bacteria (bacterial count>106/ml). Generally, it is difficult for sperm from patients with chronic bacterial prostatitis to encounter such high concentrations of bacteria. The decrease in fertility in patients with chronic bacterial prostatitis may not be a direct effect of pathogenic bacteria on sperm. Chronic bacterial prostatitis is associated with secretory dysfunction, which has an adverse effect on sperm and can lead to reduced fertility. Semen testing is complicated by the difficulty of distinguishing between immature sperm and white blood cells; Routine examinations showed a decrease in sperm count, decreased activity, increased mortality, and varying amounts of white blood cells.
Other auxiliary inspections:
Urodynamic examination: The main manifestations include decreased urinary flow rate, incomplete relaxation of the bladder neck urethral sphincter muscle, and abnormal increase in the maximum urethral closure pressure.
