"Don't speak at meetings, don't speak at small meetings, and your prostate is inflamed." This joke about middle-aged men is now suitable for young people 80 years from now. Urologists say that chronic prostatitis is common among young people aged 20-40 in outpatient clinics, often due to excessive or unclean sexual activity, resulting in local congestion or infection. Experts say that although chronic prostatitis carries the word "inflammation", the direction of treatment has shifted from the original extensive use of antibiotics to alleviate inflammation to focus on alleviating symptoms, and symptom improvement is even effective.
Prostatitis in men aged 20-40 with frequent sexual activity
According to the survey, the incidence rate of chronic prostatitis is on the rise, and the age of onset tends to be younger. "Currently, prostatitis patients account for about 50% of urological outpatients, and are most common among people aged 20-40." Liu Jiumin, chief physician of urological surgery at Guangdong Provincial People's Hospital, said, "Due to youth, strong sexual desire, recurrent hyperemia in the prostate region, and decreased resistance, they are prone to lower urinary tract infections, greatly increasing the chance of developing chronic prostatitis."
Liu Jiumin stated that acute prostatitis without timely treatment quickly turned into chronic prostatitis, which accounts for almost 90% of the cases of prostatitis. Most patients who come to see a doctor exhibit long-term and recurrent pelvic pain or discomfort that lasts for more than 3 months, as well as varying degrees of urination symptoms and sexual dysfunction, seriously affecting the quality of life of this group of men.
In addition, B-ultrasound examination of the prostate also found that some patients have prostate stones (small calcified sand in the prostate), which may also be an important reason for the persistence of pathogens and recurrence of infection.
Chronic prostatitis is not fatal, but it can drive people crazy
Symptoms such as frequent urination, urgency, poor urination, and pain in the perineum can be overwhelming in some patients, and even lead to psychiatric symptoms. Mr. Chen is a patient who is about to collapse. He is alone in business in Guangzhou, where he has a lot of entertainment, drinks alcohol, stays up late, and has irregular sexual life. During this period of time, he has general fatigue, dizziness, insomnia, and confusion. He is not in the mood to sign millions of contracts with his company. After seeing a doctor at the Provincial People's Hospital, I went through several departments before finally arriving at the urology department and was diagnosed as "chronic prostatitis.".
"In fact, this situation is not uncommon in patients with prostatitis," Liu Jiumin said. "Patients with chronic prostatitis have many symptoms, but not everyone has symptoms such as frequent urination and urgency. Therefore, sometimes they go to the neurology department or the digestive department for treatment, and then they go to the urology department. When Mr. Chen went to the urology department, he already had mental and psychological problems."
Generally speaking, 20% to 30% of patients with chronic prostatitis have significant changes in psychosocial factors and personality characteristics, such as anxiety, depression, hypochondriasis, and hysteria. Therefore, it takes a long time for these patients to completely eliminate their symptoms, and if necessary, they need to receive psychological treatment.
Early antibiotic treatment improves symptoms
The treatment effect of chronic prostatitis is not very ideal in clinical practice. Some patients have rushed to hospital and spent thousands of yuan in small clinics, but they still don't understand.
Liu Jiumin believes that chronic prostate cancer requires comprehensive treatment. In the early stages of illness (2-4 weeks), antibiotics can be used to reduce inflammation. It is now believed that a significant portion of chronic prostate cancer is still related to infection, so oral antibiotics should be taken primarily in the early stage, while symptomatic treatment should be given, such as headache and pain relief; Frequent urination and urgency are sedatives that relieve urethral tension. Next, comprehensive treatment such as prostate massage and traditional Chinese medicine can be performed.
Currently, the treatment goal for chronic prostatitis has changed direction, from the previous heavy use of antibiotics to alleviate pain and improve urination symptoms, and whether the treatment is good or not is also dominated by "symptom improvement.". To be sure, although there are many methods or drugs for treating chronic prostatitis, none of them can achieve the goal of treating all patients or alleviating all symptoms. Therefore, the treatment of prostatitis should comply with medical advice, timely return visits, and pay attention to adjusting daily routines.
