The incidence rate of male infertility is getting higher and higher, and many unscrupulous hospitals are obsessed with greed, leading to frequent treatment chaos. At the first National Symposium on Traditional Chinese Medicine Reproductive Medicine, national male experts and professors proposed seven treatment principles for male infertility, hoping to be helpful to male infertility patients.
Six Steps for Treating Male Infertility
Firstly, the age of the spouse determines the principle of treatment. Some middle-aged men come to the hospital to see oligozoospermia and asthenozoospermia, and if they want to have a child, they only know that the woman is already 48 years old when asked about the age of their partner. Director Lei Wenfeng, an expert at Chaoyang Health, said that treating male infertility must consider the woman's age. The fertility of a 35 year old woman is only 50% of that of a 25 year old woman, which decreases to 25% at 38 years old, and to 5% over 40 years old.
Secondly, we should start with the cause and strive for personalized treatment. For example, infertility caused by genital and urinary tract infections should be mainly treated with antibiotics and anti-inflammatory drugs, supplemented by drugs to improve sperm motility; For azoospermia and oligospermia, endocrine therapy with sex hormone drugs should be the main treatment; For those with low sperm motility, drug therapy to improve sperm motility is the main method; Infertility caused by varicocele, obstruction of the vas deferens, cryptorchidism, and hypospadias should be treated surgically, supplemented by endocrine and other adjuvant drugs.
Thirdly, use more experiential treatment. Through statistical analysis of over 7000 male infertility patients, it was found that 3/4 of them had unknown causes, which made doctors feel helpless, Western medicine emphasizes the need for evidence in medication, but unfortunately, the cause of male infertility often cannot be found, and there is no evidence for medication. When asked by male doctors why they use this medicine, we really cannot explain it and can only use it based on experience. However, there is only one gold standard for whether drug treatment is effective, which is to make the patient's spouse pregnant and have children
Fourthly, doctors should not be complacent about the occasional pregnancy of one or two infertile couples. Among untreated infertile couples, 1% of patients still conceive naturally every month, and 26% of patients conceive naturally within 3 years. The doctor's treatment effect must be higher than this number to be considered effective.
Fifth, medication should be used first and then IVF technology should be used. Nowadays, the technology of in vitro fertilization is becoming more and more advanced, and many infertile couples choose to have in vitro fertilization without medical treatment, which is incorrect. Improving the quantity and quality of sperm through medication to achieve natural pregnancy remains the preferred treatment for infertility.
Sixth, continuous medication should be maintained for 3-6 months. According to the production cycle of sperm, the duration of medication treatment is generally 3 months, which is a course of treatment for male infertility. Every month of treatment, a semen review is conducted to compare the quality of semen before and after treatment. If it is effective after 1-2 courses of treatment, medication can continue to be used to make the wife pregnant; If 1-2 courses of treatment are ineffective, it is necessary to consider ending drug treatment and choosing IVF technology. For severe oligospermia with ineffective treatment, drug treatment should not exceed 12 months, and it is recommended to choose IVF technology as soon as possible.
